“So… Can they go?” Blending art and Science in athlete return to play following an ACL injury

Presentation description

Tim McGrath, PhD-qualified physiotherapist, delivers a masterclass on ACL return-to-play decision making across elite sport. Drawing on his “Pitch Ready” dataset of ~1,500 athletes across professional rugby, soccer, basketball, American football, and Australian rules, Tim presents a framework that blends objective capacity and kinematic testing with the often-overlooked psychological and tactical readiness components. The talk centres on a racing car analogy, distinguishing the physical qualities that make an athlete “go” from the movement patterns and cognitive readiness that determine whether they’ll stay on the road.

Learning objectives

  • Understand why capacity-based tests (strength, hop testing) represent a minimum threshold, and why sport-specific kinematic profiling, particularly during high-speed change of direction, is more predictive of ACL re-injury than limb symmetry indices alone
  • Identify the three primary kinematic risk clusters (low knee flexion, dynamic valgus/”pepper grinder” effect, and wide base of support) and how their presence or absence differentiates athletes with ~1.5% vs ~15–16% re-injury rates
  • Apply the chaos–control continuum to design movement retraining that builds resilient, automatic motor patterns rather than consciously cued compensation strategies

About the presenter

Tim McGrath, formerly the Senior Director of Player Health and Performance at the Washington Commanders, has a physiotherapy background and a PhD in ACL, completed in 2016. His post-doctoral Pitch Ready programme has accumulated kinematic, capacity, and clinical data across approximately 1,500 athletes, professional and non-professional, spanning the UK, Australia/New Zealand, and the US. He is one of the few practitioners to have paired large-scale normative kinematic data with real-world re-injury outcomes across multiple sporting codes.
