Making it count: Criterion driven ACLR rehabilitation

Presentation description

This presentation provides an advanced, criterion-driven framework for anterior cruciate ligament reconstruction (ACLR) rehabilitation, emphasizing progression based on load tolerance, movement quality, and task-specific performance rather than time-based milestones. It integrates biomechanics, strength diagnostics, perceptual-cognitive factors, and task direction (propulsion vs. landing; vertical vs. horizontal) to explain why many athletes fail to return to sport successfully. Clinicians are guided to align rehabilitation and testing with actual injury mechanisms and sport-specific demands to reduce reinjury risk and long-term joint overload.

Learning objectives

  • Apply criterion-based benchmarks to guide progression through limb loading, load acceptance, running, and multidirectional sport tasks.
  • Differentiate between performance quantity and movement quality, particularly during landing and deceleration tasks.
  • Integrate strength, biomechanics, and cognitive load considerations to reduce secondary ACL injury risk and load-intolerant knee symptoms.

About the presenter

Dr Lee Herrington is a Chartered Physiotherapist and Senior Lecturer specializing in knee biomechanics and ACL rehabilitation. His work focuses on criterion-driven progression, objective testing, and understanding how movement quality, strength deficits, and cognitive demands influence return-to-sport outcomes following ACL reconstruction
